Dr. Pasquale and Aloha Plastic Surgery are pleased to present his new Loyalty Rewards program. This innovative program lets you earn points that can be later redeemed for services such as, micropeels, microdermabrasion, laser treatments, Botox, dermal fillers , starbucks coffee, dinners and even cash back. You may earn discounts off your next surgical procedure as well.
How can I earn Rewards points?
Simply join the Aloha Plastic Surgery & Skincare Loyalty Point Rewards Program (it’s free). Sign up at the front desk or online at www.nerve.org/rewards. We will automatically add points to your account. You can check your points online at anytime. Just log into your account !
A. You earn 1 Loyalty Point for each $1 spent on services or products.
B. You will earn special bonus points by referring another patient.
Two ways to earn for referrals:
Example 1: If your friend, Mary comes in and tells us she was referred by you --you will get credit for the referral ! The same 1 loyalty point for 1 dollar they spend will be given to you and to them. Both parties get the points first time only.
So if you refer Mary, and she has a breast augmentation, which cost 8000 dollars, both you and Mary will receive 8000 bonus points.
This only applies for the first time.
Example 2: If patient comes in for a first consultation only, but does not spend money you will still be entitled to 500 bonus points just for the referral.
C. You can earn bonus points during special promotions and incentives as advertised in our newsletter or at our Open Houses. Look for these promotions and accumulate Loyalty Points faster. See the table below for a complete listing of point opportunities.
Purchasing gift certificates for friends and family valued at $400.00 and up, you will receive 500 bonus loyalty points.

Q. Can I earn points for referring my friends and family?
A. Yes! As a token of our appreciation, 500 Loyalty Points will be rewarded for referring your friends or family members. We appreciate your loyalty.
Q. Where can I find my current points balance?
A. Your Loyalty Point total is displayed on the bottom of your receipt, or email us at ww.nerve.org/loyalty and log in with your email and password.
Q. How many Loyalty Points do I need to earn a Reward?
A. You need a minimum of 2,500 points to redeem your Aloha Plastic Surgery Rewards Program

Q. How do I redeem my points?

A. When you’ve earned enough points to exchange for a Aloha Plastic Surgery Reward you can call us and schedule the reward service of your choice and we will do the rest. Your points will be automatically deducted when you complete your appointment(s). (Please note that a minimum of 24 hours is required for cancellation of your appointment. Not complying with this policy may result in the loss of the Loyalty Rewards points that were to be redeemed for that service).
Q. Do Loyalty Points expire?
A. Yes, Loyalty Points will expire for any member who fails to earn Loyalty Points for a period of twenty-four (24) months. The participant is subject to termination of his or her membership and forfeiture of all accrued Loyalty Points.

Rules Concerning Aloha Plastic Surgery
Rewards Points
* Membership is required to begin to accumulate points in the Aloha Plastic Surgery Rewards program.
* Members earn 1 Loyalty Point per $1 appointment has been made and treatment completed (i.e., the Rewards program participant has honored the appointment and paid for the services rendered).
Terms and Conditions
* Aloha Plastic Surgery reserves the right to terminate Loyalty Points or to change these Terms and Conditions, regulations, or benefits of participation, in whole or in part, at any time with or without notice, even though changes may affect the number of Loyalty Points already accumulated. Aloha Plastic Surgery may also withdraw, limit, modify or cancel the number of Loyalty Points required for reward redemption or change the number or type of rewards offered. Aloha Plastic Surgery will have final decision should any disputes over Loyalty Points or the awarding of Loyalty Points arise.
* Aloha Plastic Surgery reserves the right to terminate a member's Rewards account and cancel accumulated Loyalty Points if Aloha Plastic Surgery believes, in its sole discretion, that the member is abusing the Rewards program
* Any Aloha Plastic Surgery rewards member who fails at any time to earn Loyalty Points for a period of twelve (12) months is subject to termination of his or her membership and forfeiture of all accrued Loyalty Points.
* Should Aloha Plastic Surgery offer rewards from any vender, Aloha Plastic Surgery will not be responsible for any vender's participation in or withdrawal from the Rewards Program.
* Loyalty Points earned by a member cannot be shared or transferred to other members’ accounts.
* Gift certificates purchased by the participant for their own use will not accumulate Loyalty Points.
* The participant’s account or accounts must be in good standing to receive or to redeem Loyalty Points.
Notice:
This program is subject to change anytime including but not limited to value assigned for rewards, complete loss of reward points if program is discontinued and the like. Therefore this is not a contract nor legal document in which you may hold us responsible for damages you may feel you incur but a good faith non binding arrangement with our patients.
